BRARS: FCC Form 605

Only certain portions of form 605 pertain to Amateur Radio.

You have two options from which you can choose: You can either download the the Form 605 that is appliciable to Amateur Radio or you may download the entire form 605 with all the schedules and instructions.

IMPORTANT: This form is ONLY to be used for direct submission to the FCC. This form 605 is NOT used by the VEC testing system. Each VEC uses its own modified version of the form. Again, these forms below can only be used for direct submission to the FCC.

Download the actual form 605, Schedule D (for vanity call sign or other change of other call sign) and all appropriate instructions. (58K)
